Description

Somerset County Council (SCC) and the NHS Somerset Clinical Commissioning Group (CCG) wish to support unpaid Carers within their role and also former Carers by jointly commissioning a Carers' support service as the needs of Carers span across health and social care. This service aims to involve Carers in identifying their needs, assisting them to continue with their caring role and ensuring their health and wellbeing outcomes are achieved.The Carers' support service is a universal service for unpaid Carers and former Carers in Somerset.
