Description
The aim of the service is to provide a range of community step-down services for forensic service users that have been discharged from acute inpatient mental health care and/or specialist secure or high dependency care. The service will be delivered in collaboration with the community forensic team.
This cohort of service users may have enduring mental health needs and a high proportion may have ongoing issues with comorbid substance misuse and non-compliance. They may also have an ASD diagnosis and / or exhibit behaviour that challenges. The provision will include high, medium and low supported housing, which takes into account the presenting needs.
This service operation will range from 24hour residential cover for people with high support needs to a supported environment with staffing presence during daytime hours, which may include a floating support provision and a concierge service at night. There will be a minimum of 26-beds across the tiered service model.
The service objective is to work collaboratively with a range of multi-disciplinary professionals to provide a rehabilitation environment that supports the forensic service user with safely transitioning through the different levels of care. The service is responsive to the changing needs of the users, enabling them to safely transition towards a sustainable recovery, while assisting them to manage their mental health and any physical health needs and achieve future independence.
The provider will deliver a tiered service model which offers a range of community supported housing provision to manage the different level of treatment needs, which will ensure:
• Relapse and re-admission is kept to a minimum
• Independence, recovery and social inclusion are promoted wherever possible.
• Improved quality of life and better insight and management of their mental health diagnosis
• Improved patient experience and systems in place to work collaboratively with service users
• The service is safe and takes into account service users and the public protection.
The contract offered is for 3 years, with potential to be extended for a further 2 years (up to 5 years in total).
