Description

NHS Dorset Clinical Commissioning Group is inviting qualified providers to express an interest to participate in supporting the design of the service model for a community persistent pain service.
The service will consist of a multi-disciplinary team that can manage a patient's physical, psychological and social needs associated with pain. It will ensure patients experiencing chronic pain are appropriately managed in a community environment.
The service objectives are to:
• To deliver a community based persistent pain service embracing a variety of techniques, including technological;
• To offer an initial triage for all referrals in line with an agreed pathway and referral guidelines;
• To have an agreed pathway and referral guidelines in place;
• To act as a Single Point of Access for patients requiring a pain service;
• To deliver an outcome based service;
• To support and empower patients through shared decision making to gain or regain self-belief and confidence;
• To work with and educate GPs to safely optimise the use of analgesia and self-management tools and techniques;

The event will be held on Thursday 22nd March at 9.15am at the Dorford Centre, Bridport Road, Dorchester, Dorset. DT1 1RR. Refreshments will be available from 8.45am

There will be a presentation followed by discussion where providers will be required to actively participate.
