Description

This is a pilot scheme to deliver information and training to the citizens of Leeds via a structured training program.Type 2 Diabetes Supported Structured Self-Management Programme for South Asian Communities is a 12 x 8 week structured education programmes for people from South Asian communities delivered to cover 3 areas of Leeds (Harehills/Chapeltown, Beeston and Gipton/Oakwood). These chosen areas are based on the highest levels of South Asian populations matched to an increased prevalence of diabetes

This is a contract in 3 lots with a total cost of 40,000 per annum.

Lot 1 - Harehills/Chapeltown - 14,000
Lot 2 Beeston& amp; nbsp; - 13,000
Lot 3 - Gipton/Oakwood - 13,000

The total length of the contract including design and the post contract evaluation will be 12 months.
