Description

The Council's vision for the Service is the provision of a Mental Health Social Care Step Down Floating Support Recovery Service that provides low level enabling support to Eligible Individuals with mental health needs. This will be to prevent, delay and reduce the needs of Eligible Individuals stepping down from social care services to enable them to recover, maintain their independence and be included in the community.
The key aims of the Service are to enable recovery, independence and social inclusion within local communities using community assets and resources for Eligible Individuals. The Service will be required to:
a) provide a motivational, coaching, empowering provision to Individuals;
b) provide individualised support consisting of a range of options that meet the needs of the individuals they are supporting;
c) work to recovery, independence and inclusion goals based on a persons' aspirations;
d) work with an individuals' assets and those of the community to support people to identify and create their own Wellbeing Networks;
e) provide a person-centred provision which promotes each Eligible Individual's recovery, independence and
inclusion, and develops their resilience to manage the disabling effects of mental illness or recurrence of illness;
f) develop strong links and referral arrangements with Adults and Children's Social Care Assessment and Care Management Teams;
g) develop strong links with the commissioned service for the provision of Improving the Emotional health and wellbeing of Children and Young People in Staffordshire;
h) develop strong links with other relevant and appropriate services to enable an effective signposting provision at any point along the Eligible Individual's journey within the service;
i) provide opportunities for volunteering and to maximise Service Delivery and the capacity of the Service
through the deployment of volunteers;
j) Involve Eligible Individuals in service design and delivery of the Service and encourage coproduction;
k) focus on recovery, independence and inclusion outcomes;
l) target and empower Eligible Individuals:
i) currently under-represented (e.g. members of Black And Minority Ethnic (BAME) communities);
ii) with contact with the criminal justice system;
iii) with additional access issues, individuals living in rural areas and young people.
The Provider will deliver a Service which offers a range of mental health support within the Contract Area
(please see Specification Document, Appendix A for a Map of Contract Area) based on an outcome focussed and asset-based approach within the community. Recovery, independence and social inclusion is achievable within communities using community assets and resources.
