Description
The Service will provide accommodation and a high quality housing support service for people with severe and enduring mental illness and those who have been discharged from inpatient wards, aged 18 years and over . The Service will provide support to individuals to engage with and access the local community, and enable them to gain the skills to live more independently and move-on to appropriate, more independent accommodation.
The Service will provide the skills and specialisms needed to accommodate the cohort currently supported at Camden Road. It will be a mental health supported living service, enhanced with C&I employed multi-disciplinary staff, including psychiatry, psychology, occupational therapy, psychiatric nursing and social work. The service will be delivered from our site at 154 Camden Road. The support element of the service will be provided by a specialist support provider and the multi-disciplinary provision arrangement formalised via a subcontract arrangement with C&I. The contracting arrangements will provide an opportunity to combine the strengths of health and voluntary sector providers into the rehab service.
