Description

The service is a programme that pioneers and develops interactive music projects for people living with dementia.
The service projects offer community and residential care based post diagnostic support for people with dementia and their carers. The outcomes of the projects for customers are to; increase confidence, reduce isolation, give a person a greater sense of identity, a sense of belonging and participation in a shared experience and improved health and well-being.

The core programme offers three interlinked strands of work:
• Music improvisation workshops for people living with dementia (8 sessions over 8 weeks).
• Experiential staff training: working with staff to enhance their understanding of personhood and the emotional needs of people living with dementia, as part of a person-centred approach to dementia care.
• Training and developing professional musicians to work in this field

Additional details

Contract Award Justification:

• Improving access to healthcare,
• Working in partnership with both primary care and Hospitals to improve access to the range of pathways for ill health prevention,
• Health promotion and community treatment options.
• When a patient requires hospitalisation the projects support efficient and effective safe discharge protocols that ensure smooth transition back in to the community and reducing the likelihood of readmission.
• Ensuring there is a strong VSC offer strengthens our local community

It is anticipated that this not only improves health outcomes for this vulnerable group but may also reduce costs incurred by avoiding extended hospital stays and improving patient flows.
