Description

Health and social work services - Falls Prevention. The service aims to prevent residents from falling, falling again or fracturing from a fall by providing a range of services that will prevent falls and assist with their mobility. The requirements are split into 2 lots; Lot 1 - Exercise and Balance classes / Lot 2 - Home Risk Assessment. Bidders may bid for either Lot 1, or Lot 2, or both lots, indicating clearly within the Form of Tender which lot(s) as to which lots are being bid for. The appointed bidder (or bidders - the term 'bidder' is generally used within this document) will provide specialist falls and fracture prevention interventions for all adults aged 65 years and over in the Borough of Bexley. This will be a pilot for 12 months with the option to potentially extend. The pilot will be evaluated at month 9 to determine the success and future models.
