Description

South Gloucestershire Council is a Unitary Authority in the South West of England. The Council currently commissions a variety of services for adults and children. The Shared Lives Service provides adults with the opportunity to stay in the homes of individually recruited, trained, and approved hosts and their families in the community, known as Shared Lives Carers. This can be for a long or short-term basis depending on assessed need and outcome. The Shared Lives Service is tailored to meet the needs of the adult being placed with the host and will aim to support with developing independence. The role of the Shared Lives Service is to support and enable the Shared Lives Carers to provide high quality care and support to the adult placed with them.

The Provider of the Shared Lives Service will support the Shared Lives Carer host and their family throughout the placement and will ensure that they provide the level of care and support that has been agreed and supporting individuals towards identified outcomes as part of their care and support planning.
