Description
This Notice is to notify the market of the Contract Award for the Dementia Post Diagnostic Support Pathway by NHS Midlands and Lancashire CSU on behalf of NHS Worcestershire CCG's.
The purpose of the procurement was to secure a comprehensive community-based Dementia Post-Diagnostic Pathway for Worcestershire residents.
The service will aim to provide a high-quality service for patients who are registered with Worcestershire CCG GP Practices.
The service will be based on best practice and will provide appropriate assessment, community services support, follow up treatment, review and discharge.
The service is essential to inform and enable people with a diagnosis, and those who support them, to live as well as possible with dementia and prepare for the future.
The current service provision for Dementia Post Diagnostic Support has been reviewed. Commissioners propose to procure a single service to provide all elements of the existing contracts, aligning with primary care / neighbourhood team structures to ensure a more targeted provision based on population need.
The provision of support that has been identified includes:
- Peer Support/ Dementia Cafes
- Dementia Information Hub working with existing local infrastructure
- Drop in services
- Access to professional clinics
- Appropriate support for people with Young Onset Dementia (YOD)
- Regular engagement with carers to inform service delivery/developments
The service provider(s) will be required to work collaboratively with existing providers as part of an
integrated, evidence based approach to patient care.
The CCG's anticipate that this procurement will be
awarded to one service provider or a lead provider on behalf of a consortium.
The budget available for this service is £310,000.00 per annum, including CQUIN. Bidders are expected to provide an innovative solution and make best use of the budget available.
The Contract Period will be for 3 years with the possiblity of a 1 year extension, NHS Standard Terms and Conditions will apply.
