Description

In alignment with the Department of Health framework "Integrated Care and Support; Our shared Commitment" dated May 2013 , NHS Enfield Clinical Commissioning Group (ECCG) will be seeking to transform local services to be able to deliver more joined up, seamless and aligned personalised health and care services for older people, their families and support network ,and improve patient outcomes.

This particular programme of change, of which ECCG is commissioning external advocacy support for, seeks to meaningfully involve patients and their families in the planning process to transition a cohort of patients who have been supported on longer stay Continuing Health Care (CHC) wards over a number of years; back to the community to lead rich and fulfilling lives Enfield CCG are committed to ensuring that these patients and their families are empowered to actively contribute towards discharge planning process and are kept well informed about their rights in terms of decision making so that discharge planning and after hospital care remains person centred, seamless and well-co-ordinated.

Enfield CCG is looking to commission an external advocacy service to empower (approximately 30) patients who have been supported on CHC longer stay wards for a number of years and their families to support the transition and discharge planning process. We are seeking to commission a suitably qualified provider with appropriate experience who must be positioned to deliver advocacy services to a wide range of service users.
