Description
The Family Weight Management Service will:
• Support overweight and obese Children and Young People (CYP) to reach and maintain a healthier Body Mass Index (BMI).
• Offer a range of person-centred, family focused weight management programmes. Adopt flexible programmes that allow CYP and Families to access the Service quickly and easily.
• Offer a range of delivery methods e.g. face to face and digital, for CYP and Families of different ages and at different stages of development.
• Meet the needs of all CYP particularly with Special Educational Needs and Disabilities (SEND) or English as a Second Language (ESL).
• Multi-component approach including nutrition and food related education, fun physical activity participation for all, behaviour change information advice and guidance, positive parenting skills.
• Provide the Families with Information, Advice and Guidance (IAG) and facilitate access to available online self-help tools.
• Develop an agreed Goal Based action plan for each CYP and their Families which compliments dietary behaviour change and needs that CYP may already need to follow and/or have.
• Undertake a review of all CYP and their Families on a regular basis to ensure that they are meeting the Goal Based Outcomes set within their Goal Based action plans.
