Description

Monies to enable implementation of interventions: £25k per participating LCP during the 20 weeks PHM Programme. The funds will support intervention work; for example, extended consultations for identified people where a targeted intervention is appropriate or regular extended multi-disciplinary team meetings to support integrated working. As a pilot LCP you will be in a strong position through any initial intervention testing to support 'Fast Follower' LCPs to take forward tried and tested intervention methods.
