Description

Essex County Council (ECC) are seeking a Provider to develop and deliver a service for people living withdementia and their families & carers across Essex. The service will work in partnership with professionals across the health and social care system in Essex, whilst taking into account local differences, to provide seamless access to coordinated information and advice to enable people living with dementia, and those that care for them to live well, and independent for as long as possible.
The service will have two separate elements which link closely - the community support and a family information, support and navigation function. Initially there will be a connecting role, the Dementia Co-Ordinator who will prove the link between the elements, the community and local implementation.
