Description

The NHS is under significant pressure to continue providing high quality care to Londoners in light of increasing & changing demand, constrained funding and significant workforce challenges. The national service improvement strategies described in the 'NHS Long Term Plan' (LYP), 'General Practice Forward View' (GPFV) and 'Next Steps on the NHS Five Year Forward View' (FYFV) provide a unique opportunity to transform London's Primary Care and Urgent & Emergency Care services.
Support offered to STPs to deliver NHS England transformation priorities has evolved over time, and tended to focus on Urgent & Emergency Care (U&EC) or Primary Care separately. There is an ambition to remove complexity and duplication from the system, reduce costs and improve coordination across STPs & CCGs to improve patient experience and access to quality care.
The continuation of a Digital Transformation Team (DTT) reflects our recognition in the value of harnessing innovative technologies to simplify patient interaction with the most appropriate health and care services across London when they need it in a more coordinated manner.
