Description

Blackburn with Darwen (BwD) Council aims to improve and protect the health and wellbeing of the local population with an emphasis on reducing health inequalities. Public Health commission the local NHS Health Check programme which was originally launched in 2009, as mandated by the Health and Social Care Act (2012) and funded through the ring-fenced Public Health Grant. The purpose of this programme is to prevent stroke, kidney disease, heart disease, type 2 diabetes and dementia across the population and within high risk and vulnerable groups.

The NHS Health Check provides people with an opportunity to discuss their risk factors and their likelihood of future disease, including a risk-score. It is an important step in encouraging people to try and reduce their risk of cardiovascular disease (CVD), through lifestyle changes and signposting/referral to services, such as BwD's Wellbeing Service and medication where appropriate.
Our NHS Health Check programme is aimed at people who are registered with a BwD GP practice, are aged 40 to 74 years and are without existing CVD (as specified by the national guidance) who have not received an NHS Health Check in the last 5 years.
