Description

This opportunity is part of a pilot project. The Council is setting up a project team to focus solely on reviewing 490 (approx.) customers over the course of one year, with a view to supporting 87+ of these to become completely independent. Working with the Council and the Council's staff on the Focused Reablement Project, the supplier shall provide Occupational Therapists to undertake the reviews.
