Description

The Reablement Service forms part of the Intermediate Care Service (a range of integrated services), and supports the promotion of independence and faster recovery from illness. The Service aims to prevent unnecessary acute hospital admission, premature admission to residential care, support timely discharge from hospital, to minimise, delay or avoid the need for long term domiciliary supports and to maximise independent living.
The Reablement Service is therapy led, with occupational therapists leading and managing the Service. Each Period of Reablement will be focused on the achievement of clearly defined outcomes set by the Service User and their Allocated Worker.
The Service will be short term support and will involve intensive reabling / enabling support for adults. Services will have a strong focus on the prevention and wellbeing elements of the Care Act 2014. There will be an ethos of enabling rather than 'doing' for Service Users, supporting individuals to achieve outcomes and goals.
Providers will be paid on a Block Contract arrangement as detailed below. The Block Contract guarantees payment for a specific number of service users for the successful Service Providers.

The Service will support a minimum number of Service Users each week as determined by the Authority.

The Block Contract hours paid by the Authority will represent 37 hours support to be delivered by the Service Provider per Service User. This level of support per Service User will be known as the Reablement Package.
The actual number of support hours delivered by the Service Provider for each Service User of Reablement may be higher or lower depending on the needs of the Service User.
The procurement will include three lots:
Lot 1 North Lancashire
Lot 2 Central Lancashire
Lot3 East Lancashire
Tenderers may bid for one or more Lots. The maximum number of Lots that may be awarded to one Tenderer is one.
