Description
London Borough of Tower Hamlets is seeking a number of providers to provide a range of semi-independent and supported accommodation services for young people in care aged 16 and 17 and Care Leavers aged 18 to 24. Successful provider will work with young people to build a range of life skills which enable them to make a successful transition from being looked after to becoming a care leaver and then on to living independently.
The framework is made up of 4 lots as follow:
Lot 1 Assessment, Support and Mediation 16 to 25 year old to provide support for the above age range, from two different routes. 24 hours support from the homelessness route and the second from the care leavers route.
Lot 2 progression support for 16- and 17 year old
This service will provide support for above age range, services will be staffed 24 hours of the day throughout the year, which will enable the service to support elevated risk young people. The number of hours of support will have core and flexi elements, meaning that they will receive a minimum number of hours of support as a baseline, and access more hours of support if needed.
Lot 3 progression support for 17.and 21-year old
This service is similar to the above lot, however there will be more placements where the service will be staffed only during the day, therefore supporting young people with mixed support needs.
Lot 4 Transition and move on
This service will be for young people with low support needs, there will be no staff presence during the day, with young people accessing support through a floating support service. This service will be the last step for a young person before they are moved onto general housing, where they no longer need to access the support from this service.
