Description

The service aims are the delivery of bespoke packages of continuing care for those children and young people under the age of 18 who have continuing care needs that cannot be met by existing universal and specialist services alone.

- To provide continuity of care through the provision of consistent and reliable care staff.

- To undertake a comprehensive assessment to facilitate the timely implementation of a care package. The assessment will set out the delivery of care required in partnership with the child/young person and family, ensuring that the 'voice of the child' is at the centre of planned care.

- To deliver bespoke care packages in accordance with an individual care package agreement commissioned by the Clinical Commissioning Group.

- To work in partnership with children/young people /families, services and commissioners in order to deliver an effective package of care as outlined within the individual care package agreement.

- To support the child / young person to live at home with their family and within the wider community.

The scope of Procurement as detailed in the service specification for this service has been developed based on the needs of the CCG's

Services provided under this agreement will include: Children's and Young People's Continuing Care

Care Package Delivery

The service is required upon request to indicate their ability to deliver an identified care package by responding to the published 'Care Package Request'. The Service will contact the child/ young person/family within 2 working days of the notification of the care package being awarded. The service shall implement the care package within the agreed time frame outlined in the care package request/agreement.

The Service shall make available to the child, young person and family an up to date statement of purpose setting out the aims, objectives, and philosophy of care and parameters of the service provided.

The service will be underpinned by the following principles:

- High quality, bespoke trained competent staff
- Continuity and reliability of care through a bespoke care team
- Collaborative working, proactively involving children, young people, families in service delivery.
- Equitable, prompt implementation of community based care to facilitate safe discharge from hospital and/or community
- Comprehensive multi professional/multi agency understanding of need and delivery of care.
- Care planning that is personalised to meet individual need and outcome focused , to reflect the child/young person's Education, Health, Care Plan.

Pan Essex AQP Children and young People's Continuing Care (CYPCC) Framework since 2012. Pan Essex CYPCC covering - 7 Clinical Commissioning Groups - Working across 3 Sustainability and Transformation Partnerships - Working with 3 Local Authorities - DOH (2016) National Framework for Children and Young People's Continuing Care.

Contract duration 3 years with the provision to extend for a further 24months.
