Description
NHS Berkshire West Clinical Commissioning Group on behalf of NHS Frimley Clinical Commissioning Group are seeking to commission a service to support autistic people and their families/carers across Berkshire.
The Pre and Post-Diagnostics service for adults with autism seeks to work collaboratively with key stakeholders and co-produce a new service.
The service is for those aged 18 and over who are registered with a GP within Berkshire and will offer support (at any stage before and after diagnosis) to enable autistic people to live healthy and fulfilling lives in the community and achieve the following outcomes:
• Improved outcomes (e.g. mental, and physical health, quality of life, wellbeing, understanding of autism, autonomy, and independence) for this group.
• Reducing loneliness and social isolation.
• Building resilience.
• Contribute to the ambition to reduce unnecessary hospital admissions.
• Early help to reduce the need for more intensive support services later on.
The successful provider will design, promote and deliver accessible and practical support with the aim to:
• To improve quality of life, independence and reduce health inequalities for autistic adults in Berkshire.
• To deliver autism pre- and post-diagnostic support to autistic adults, carers and their families in partnership with health partners and local authorities, further developing best practice with national and local third sector organisations working with adults diagnosed with autism.
• To work in partnership with health and social care commissioning organisations to support the commissioning and delivery of accessible, person-centred high-quality support for autistic adults.
The contract is £80k per annum and will be for an initial 2 year term with an optional extension of any period up to 2 years.
This procurement is being carried out by NHS South, Central and West Commissioning Support Unit (SCW) on behalf of the Commissioners.
