Description
Stoke-on-Trent City Council are seeking to commission a Service Provider to deliver an accessible tier 2 lifestyle family weight management service (FWMS) called Active Families (AF), which supports overweight and obese children to reach and maintain a healthier BMI.
The service contract is for a maximum of 12 months subject to review with a maximum of £48,000 available for the 12 months for 100 families. The Provider must ensure that the Active Families programme is available across the City of Stoke-on-Trent with an even distribution of programmes in the north, central and south of the city. All responses to the service specification will be evaluated against their evidence of the criteria set out in the Quality Standards. Providers of AF will be required to demonstrate competencies and ability to deliver educational sessions on nutrition and healthy eating and physical activity sessions that cater for the age range 4- 11 years and the whole family.
