Description

Islington Council invites suitable expressions of interest from suppliers for the provision of the Islington Carers Hub Service. Current status / Background The aim of the contract is to ensure the provision of a range of high quality services to carers who care for a resident within Islington. The current contract ends on 30 September 2015. Services currently provided are: Advice, information and signposting Identifying hidden carers Outreach Support services Strategic Leadership Carer engagement. The Requirement The Provider will: Work with adult carers who care for a resident within Islington Provide an office base in Islington for staff to operate from Work flexibly so that carers can access Hub services digitally, in a wide range of community venues, and outside usual office hours Act as a single gateway to a range of services for adult carers, referring and signposting carers as necessary to meet their needs Provide strategic leadership on carers' issues within the borough Deliver impartial advice and information to carers Provide a range of support services to carers enabling them to achieve income maximisation, achieve well-being as outline in the Care Act 2014, clause 2.1.2, and complete forms Set up and administer support groups as needed Review, develop and deliver an annual flexible breaks fund designed to support carers in their caring role Undertake carers assessments and support planning for carers in need of low level support where neither the carer nor the cared for are in receipt of a Council care package Work proactively to ensure smooth transition for young adult carers Deliver providers forums, presentations, workshops and focus groups Work effectively to support carers into employment and education Deliver and coordinate Carers Rights Day and Carers Week activities across the borough annually Work effectively to identify hidden carers and engage them with services as appropriate Ensure that carers are actively engaged in service developments and appropriate areas of service delivery Pursue fundraising opportunities, in partnership or alone, to develop service areas, commission research or undertake other activities as agreed with the authorising officer.
