Description

County Durham and Darlington NHS Foundation Trust Procurement Department (County Durham Procurement Consortium) is facilitating this tender on behalf of:
NHS Sunderland Clinical Commissioning Group (the Contracting Authority)
The primary objective of the Contract is to procure a medicines optimisation practice support service that will deliver the Sunderland CCG medicines optimisation agenda in practices whilst also expanding the pharmacist's role and allowing some flexibility to respond to the needs of primary care.
The contract duration is 24 months from Go Live with the option to extend of 1 x 12 months
