Description
The NHS Health Check service contract will be let to any qualified provider for an initial term of 3 years, with the option to extend for up to 2 further years at the sole discretion of the East Riding of Yorkshire Council. The purpose of the NHS Health Check service is to assess eligible East Riding residents aged between 40 and 74 years inclusive, on their future risk of cardiovascular disease and then provide tailored face to face feedback including healthy lifestyle advice and referrals to further Public Health services when deemed appropriate.
The Health Check service will be paid at a fixed rate of £26.00 per completed Health Check for eligible residents of the East Riding of Yorkshire. Monitoring will be done via the Outcomes4Health/PharmOutcomes system.
