Description
NHS Northumberland Clinical Commissioning Group proposes to engage with the wider market to inform future service model development and/or commissioning intentions for the provision of a Joint Musculoskeletal and Pain Service (JMAPS) for the population of Northumberland.
The objectives are to:
Provide clinical assessment, treatment, medicines management and patient navigation within a comprehensive community service for patients with MSK and pain problems.
Enhance the management of patients within the community and actively manage the demand for secondary care services ensuring patients have prompt access to appropriate treatment and are supported to self-manage where appropriate.
Identify patients who are appropriate for surgical intervention and make onwards referral.
Work closely with specialists, in the relevant sub-specialities, to ensure effective care for patients and develop agreed models of care for common musculoskeletal conditions and pain management.
Support the referring clinician / health professional in identifying and actively managing patients at high risk of chronic pain and support the patient's needs based on lifestyle.
Use shared decision making (SDM) to deliver support to patients enabling them to manage their condition. This will include supporting the external SDM process for patients who may require major joint replacement surgery.
Enable patients to take ownership and to adopt strategies of living, which allow them to lead as fulfilling and independent lives as possible with less reliance on the health care system.
Work with GPs to increase their knowledge and confidence of diagnosis and management of MSK and pain conditions.
Encourage collaborative and cross boundary working with other contracted parties.
Maximise technology such as websites and Apps to help patients to manage their condition.
