Description
The Manchester Citywide Support Service will deliver a single holistic non-medical, person centred low level, cross tenure housing and customer support service which will promote independence, choice and wellbeing. The service will facilitate hospital discharge and integrate with health and care services to help prevent hospital admissions and readmissions. The service to be delivered will comprise the following aspects:
• Provision of a Handyperson's Service including a Dementia Aids and Adaptations programme and Child Accident Prevention Programme
• Home Repairs Support service (including grants and low cost loans) and also including a boiler repair/replace service
• Provision of an all-encompassing non-clinical Hospital Discharge Support Service; Administration of a Home Support Fund
