Description
NHS Ealing CCG is seeking expressions of interest from suitably qualified and experienced healthcare providers who would like to be considered for the provision of Ealing Community Ophthalmology Service.
The reason for this procurement is the imminent expiry of the current service contract. The new service contract will commence in April 2018.
The re-procurement will provide an opportunity to re-model the clinical pathway to improve efficiency, update the service specification to enable more patients to be supported in community settings. This will enable patients to be seen closer to home, resulting in improved outcomes and reduction in variation in care.
Across North West London, commissioners and providers are currently working together to consider what accountable care partnership/s could look like. The provider of the Community Ophthalmology service will be expected to participate in the discussions to develop this model of care and consider how it could operate within this model. Alignment to this vision of healthcare delivery is important for the future of health and care for Ealing residents.
The service will include (but not limited to):
• Single Point of Access for triaging, assessment, investigation, monitoring and management of eye conditions for GP and optometrist referrals.
• Access to SystmOne for community provider to ensure seamless referrals and information flow along care pathway.
• Reduce the length of cataract pathway through the management of pre and post- operative cataract patients in community settings.
• Monitoring of stable and low risk patients with glaucoma, ocular hypertension and those requiring repeat pressure measurements.
• Management of long term conditions and supporting self-management.
• Facilitating direct referrals to Hospital Eye Service where appropriate.
The provider will be expected to work alongside other ophthalmic providers to develop protocols for patients that can be appropriately and effectively managed in a community setting.
The service must be able to address the needs of patients who require immediate access to services (within one working day) in a timely manner to avoid unnecessary appointments in acute settings.
