Help2Change want to procure computer software, to run across 43 GP practices in Shropshire and in community locations, to aid in performing NHS Health Checks, and to perform reporting of aggregated, non-identifiable data.The system must be available for call off under GPSoC lot 1 for GP practice based elements (or have commenced assurance activities under GPSoC lot 1).
