Description

The Council wishes to commission a cost effective Home from Hospital service
to support people with discharge from hospital and prevention of admission to hospital. The aim is also to enable service users to regain their confidence after a period in hospital or a short stay in A&E in a way that will help to promote their independence, reduce dependency on intensive home care packages, and reduce the likelihood of hospital readmission or long term care.

The contract will be awarded initially for one year with a possibility to extend for a further one year, depending on performance and availability of funding.
