Description
NHS Arden and Greater East Midlands CSU (AGCSU) on behalf of Mansfield & Ashfield and Newark & Sherwood CCGs (referred to as the Commissioner(s)) is inviting suitably qualified and experienced providers to deliver the Service(s).
An Acute Home Visiting Service provides support to General Practice in delivering quality care through Proactive General Practice by centralising aspects of daily provision. The initial patient contact remains within General Practice through a triage and referral system, maintaining continuity for patients. This service will provide optimal care for patients keeping them at home wherever possible, maximise efficiencies of workforce, and demonstrate value for money by reducing non-elective admissions.
