Description
NHS Waltham Forest CCG is seeking offers for the provision of Community direct GP Access ambulatory ECG,BP and Standard 12 Lead ECG services under the any qualified provider (AQP) process (Lot 1).
In addition, NHS Waltham Forest clinical commissioning group will be opening the opportunity for new qualified entrants for the provision of:
Community direct GP Access non-obstetric ultrasound (NOUS) Services (Lot 2), and;
Community direct GP Access Echocardiogram services (Lot 3);
under the Any Qualified Provider (AQP) process. The anticipated period of contract is 23.8.2018 to 22.8.2021,with a potential option to extend for a further 2 year period.

The lowest/actual value of GBP 780 000 above, represents the cumulative total over the initial 3 year contract period for the lowest value lot, while the highest value shown represents the cumulative total over a maximum possible 5 year contract period for all the service lots - Lots 1, 2 and 3. (i.e. inclusive of the initial 3 year term plus a possible 2 year extension)
The deadline for submission of AQP response documents for all Lots is 12:00 pm on 19.03.2018. Bidders may submit bids for 1, 2 or all 3 of the service lots.
The relevant policies and contractual documents will be available on pro-contract (https://procontract.due-north.com/Login) along with the AQP documents. There is a messaging facility available on the pro-contract portal to send any clarification questions in respect of the offer. Bidders must use pro-contract messaging to submit clarification questions. Clarification questions received by other means will not receive any response.
