Description

NHS Midlands and Lancashire Commissioning Support Unit (CSU) are working on behalf of NHS East Lancashire CCG who wish to procure a Community DVT service for up to 18 months as a local service pilot for patients living in the East Lancashire areas (Hyndburn, Ribblesdale, Burnley, Pendle and Rossendale). This contract will be funded on a cost per case basis with a dedicated prescribing budget.

East Lancashire CCG would like to commission an innovative and Integrated Community DVT Service which provides a one stop shop approach and streamlined pathway across dedicated sites in East Lancashire. The pilot will provide increase accessibility and equitability via a high quality, safe and streamlined pathway, providing which is better value for money and is responsive to patients needs and predicted demand in line with NICE guidelines.

The following key service requirements and outcomes:

Service requirements:

• 5 day service, delivered from 8am to 8pm (Monday to Friday) The service will provide whole leg scan.
• One service pathway with a referral process linked to EMIS for a streamlined patient pathway to achieve efficient and safe patient referral and management.
• Service to manage the full patient journey in compliance with agreed local care pathways and protocols which includes up to 3 months patient management (this will involve initial and follow up appointments, prescribing, full leg ultrasound scan, self help advice and education) in compliance with NICE recommendations for unprovoked DVTs as well as other related conditions causing leg swelling redness and pain including thrombophlebitis, cellulitis, ruptured bakers cysts, superficial skin infections etc. Unconfirmed DVT patients will be discharged back to their regular GPs for further review and treatment.
• Provide prescribing and ultrasound scan to 100% of referred patients within 4- 24 hours of being requested by a health professional.
• Agree a local pathway from A&E/UCC/Elective Treatment Centre to deflect non-emergency/complex patients to community DVT Service. This is in support of managing demand and flow from A & E and managing patients closer to home.
• To actively promote and encourage the utilisation of the service across all Pennine Lancs GP's through local campaigns and communication.
• To refer discounted DVT patients onto other community service pathways for longer term management e.g. cellulitis pathway/healthy legs café.
• To refer all complex patients to secondary care (via agree protocols and pathway). These pathways have been agreed at Blackburn with Darwen and it is expected that they will be replicated across East Lancashire.

Service outcomes:
• Service to manage an approximately 443 patients per annum.
• 100% of patients offered diagnostics including an ultrasound scan, prescribing, diagnostics and patient appointment within 4-24 hours to ensure the service is delivered within NICE guidance.
• To increase the number of patients with suspected DVT's diagnosed, assessed, treated and managed within a community setting.
• Service to manage 95% of patients referred/deflected from GP's/A & E/ UCC/Out of Hours service seen per annum.
• Improve the patient experience, through patient surveys and feedback measured via the Quality reporting via the schedule 4 contract report, delivered by a standardised local pathway to reduce variation in service.
• A reduction in the number of confirmed and suspected DVT patients accessing secondary care of approximately 85%, which will be achieved through deflection from GP's and A &E/UCC (see Appendix 3 for service activity and cost modelling).
• Provision of a cost effective and efficient service which delivers value for money via 6 monthly service evaluations.
