Description
This is a Request for Information (RFI) published on behalf of NHS Suffolk and North East Essex Integrated Care Board (ICB).
With this RFI Suffolk and North East Essex ICB is seeking to learn the level of possible interest from organisations in relation to supporting and facilitating the development of Health Inequalities (HI) Plans for GP practices across the North East Essex Health and Wellbeing Alliance. The same information will be gathered from different organisations to inform our service model and future procurement plan.
The ICB is seeking to commission a high quality service and organisations should have the ability to meet or demonstrate the following:
- Knowledge and understanding of the role GP practices have within their communities in the context of Primary Care Networks and Neighbourhoods
- Robust project implementation strategies capable of delivering high quality plans in a timely manner
- Experience and understanding of how to mobilise teams working in environments with competing priorities
- Able to empower end users to deliver defined outcomes and deliverables
- Offer solutions to GP practices that mitigate and resolve factors that risk them fully participating in developing HI plans, as set out, in the Tackling Neighbourhood Health Inequalities service requirement of the PCN Contract Network DES. This may include:
o The identification, understanding, and use of information that recognises populations experiencing inequalities
o Ensuring the voice of its local population is heard in agreeing population engagement plans
o Maximising the outcomes of its local population so there is sufficiently represented views on the gaps and barriers to care
o Negotiation and influencing skills to define an approach that identifies and addresses the unmet needs of this population.
- Providing subject matter expertise for the design and creation of measurable outcomes. The Tackling Neighbourhood Health Inequalities service requirement of the PCN Contract Network DES expects, "locally defined measures agreed with local commissioners in line with, and co-ordinated between, wider system strategies to tackle drivers of inequalities." Organisations identified as part of this process will need to empower GP practices to participate in those conversations so outcomes being measured as relevant and meaningful for their local population.
Organisations will be expected to support GP practices to deliver on the HI Plans ensuring co-ordination and an integrated approach.
