Description

NHS Southwark Clinical Commissioning Group (the CCG) is looking to commission a community based patient outcome focussed tier 2/3 weight management service for adults in Southwark.

Lead Providers are invited to a market engagement event .The annual contract value is anticipated to be in the region of £250,000 per year, with an initial contract duration of 2 years, and with a possible option for contract extension thereafter. The CCG is keen to work with the provider during the period of the contract to test and learn from approaches to enable on-going service development and improvement. Therefore this service will be subject to careful analysis, evaluation and on-going development, and allow existing providers who are currently delivering weight management interventions in the Borough to work together, either informally or via formal partnership arrangements, and be key players within the new, comprehensive service. It is intended that the service will then be either re-procured, informed by the learning from the initial 2 year, or the contract may be extended in line with the extension clause (if any) within the contract. The CCG will work with providers to re-shape the service, and anticipate eligible patients would be referred by their GP or practice nurse.

An Obesity Specialist nurse would conduct a triage telephone call and triage into the relevant Tier, service or initiative. Patients would have regular specialist nurse or dietician review as well as access to health trainer, psychologist, and medical physician appointments plus individual and group exercise and healthy cooking classes would be agreed between clinical team/patient. Our aim is to deliver better outcomes for patients, so they feel more in control of their weight management and enjoy better health and quality of life. This will contribute to CCG priorities: Healthy life expectancy, Reduction of health inequalities, Increased 'patient activation' so more people are engaged in their healthcare, & More patients reporting a better experience of healthcare services Supporting people to manage their weight more effectively offers huge potential to improve the health and wellbeing of our residents. Targeted support will help to reduce inequality.
