Description

Delivery of an Ophthalmology Outpatient Glaucoma Service. It is expected that this service will be delivered within the Ophthalmology department at Queen Elizabeth Hospital, Birmingham using UHBFT equipment, IT systems and processes. It is expected that the clinics will run on Sundays and at least one evening per week.

The contract will be for an initial period of 12 months whilst the CCGs commission a community based model. The contract will include an option for UHB to extend the contract for a further 12 months.

The successful provider will triage and clinically assess all new Glaucoma outpatient referrals, undertake clinically appropriate diagnostics and treatment (excluding surgical interventions) and then onward refer or discharge as appropriate. The contract will include follow-up appointments for these patients for the contract period.
