Description
The service will facilitate the safe transfer of care for service users who are medically fit for discharge from acute mental health inpatient units. It will be available for 5 people at any point in time. The focus will be to support access to suitable accommodation and assistance to maintain a tenancy. The service will be provided for a maximum period of 28 days for service users meeting the eligibility criteria; any exception to the length of stay will be by mutual agreement between the provider and commissioner and reported as exception in the quarterly performance report.
Value of contract: Maximum Contract value 110,800 per annum
Term: 4 years with 1 year with possible extension

4. Responding to the tender - There will be a tab allocated to any tender stage (e.g. PQQ and/or ITT). Under the `My Tender Return` area, you will be required to complete any mandatory questionnaires and upload any requested documentation. You can now use the `Correspondence` function to communicate with the buyer and seek any clarification (any clarifications will be found within the `Clarifications` tab). - Note the deadline for completion, then follow the onscreen instructions to complete and submit your response. - There may be a mixture of online and offline actions for you to perform (there is detailed online help available).
You must then submit your reply using the `Submit Response` button at the bottom of the page. If you require any further assistance please consult the online help, or contact the In-tend Support Desk. Alerts will be sent to your request email addresses when any correspondence, alterations or clarifications are issued.
Please note the ITT and procurement documents will be published on this site once the advert appears in the Official Journal of European Union, the advert was sent for publication on the 25th September and thus this should be in the next 3-4 days.
