Description

North Lincolnshire Council are seeking to commission a service provider to deliver specialist regulated care for individuals with a range of complex needs including, Learning Disability, Autism, Asperger's, and Physical Disabilities and Mental Health needs.

The Poplar Tree Avenue development consists of 20 specially adapted homes (12 apartments and 8 bungalows) designed to meet the needs of disabled and vulnerable people, enabling independent living in the community with support. See Appendix A for information regarding the site, including anonymised pen pictures of potential residents and phased move in details.

The service provider shall be able to meet the differing needs of the people entering the development throughout the duration of this contract. There shall be an expectation that the service provider will ensure competent staff are recruited and undertake full training requirements to support the people in this service

The service provider shall also ensure a core service provision to all within the site that continues to develop and enhance the care and support for the people within the service, recognising that in some instances individual choice may mean they bring existing care services with them for their main care needs (although this is expected to reduce over time).
