Description
Wiltshire Council is looking to commission discharge to assess beds to support Salisbury Hospital, Royal United Hospital and Great Western Hospital over the winter months.The purpose of discharge to assess beds is to provide an alternative to a person making long term decisions in a hospital environment, but instead providing a care home setting where socialcare assessments and onward discharge planning can be carried out. D2A is a particular model, which includes an early discharge expectation, Reablement goals, a link social worker and occupational therapy, alongside a limited period of admission up to 6 weeks maximum.
