Description

Expressions of Interest - Life After Stroke Services DHSC.753
The Isle of Man Department of Health and Social Care ("the Department") invites expressions of interest from experienced providers to provide a service to stroke survivors living in the Isle of Man and to assist individuals in managing the symptoms of their condition and adapting to the changes in their life style as a result of experiencing a stroke.
The Contractor will ensure stroke survivors and their carers have access to appropriate information and education in stroke care and rehabilitation; and that individuals are given the opportunity to communicate with peers and are able to contact specialist professionals for advice when necessary. The Contractor will work collaboratively with the Department to provide an integrated life after stroke programme to patients and their carers.
In carrying out the services the provider will be expected to:-
- provide guidance, information and emotional support to stroke survivors and their carers in post stroke rehabilitation.
- support the carers / families of individuals who have experienced stroke in accepting and adjusting to the changes in their life style.
- actively engage with stroke survivors; working to improve the quality of life, building confidence and improving morale.
- work to reduce social isolation for stroke survivors and their carers.
- recognise the need for other health care professionals and refer individuals when appropriate.
- support people who have communication difficulties as a result of experiencing a stroke.
- recruit and train volunteers to provide additional support where necessary.
- provide secondary health education advice to stroke survivors and primary advice to the carers about future stroke prevention.
- provide informative supporting media and materials to stroke survivors, carers and the Department.
- offer and / or support, where appropriate, a return to work scheme to each service user as part of their package of care.
