Description

Kingston upon Hull City Council and NHS Hull Clinical Commissioning Group ("the Commissioners") are seeking to commission a Homeless Health Service through an open procurement procedure. The preferred Provider will be required to demonstrate how they will deliver a clinically safe, high quality, dignified, and cost-effective service that puts the individual at the centre of their own care and work to transform health outcomes for one of the most vulnerable and deprived groups in society.

The key principles of service delivery are to:
• Provide a seamless pathway for individuals who are homeless who present in hospital and those who are discharged (hospital-based intervention); and who are identified in the community who have unmet health needs (community intervention);
• Work in a trauma informed way, creating a trauma informed environment and led by a trauma informed and responsive organisation;
• Provide pro-active and personalised care to people to homeless people;
• Improve access to appropriate health and social care services for people experiencing homelessness;
• Improve the experience of health and social care services for individuals who are homeless;
• Liaise and work with key local partners, stakeholders and services to support the health and care interventions provided to individuals who are homeless.

Note 1. Value of contract
Potential Providers are to note the estimated maximum contract value includes funding for the Community Intervention element of the service, which is subject to, and dependent on, an annual grant funding application being submitted by Kingston upon Hull City Council to the Ministry of Housing, Communities and Local Government - Rough Sleepers Initiative (RSI) fund being approved.

Should an annual grant funding application not be approved by the Ministry of Housing, Communities and Local Government, this element of the service would not be expected to be delivered, and the annual contract value would reduce accordingly.

Note 2. Hull's Preventing Homelessness and Rough Sleeping Strategy
As a key priority under Hull's Preventing Homelessness and Rough Sleeping Strategy, Kingston upon Hull City Council and NHS Hull Clinical Commissioning Group have been successful in securing funding from a range of sources to support rough sleepers and those at significant risk of rough sleeping. The total funding secured is approximately £2.2m per annum with the potential for this to increase through newly available funding. In order to ensure a cohesive and strategic approach to addressing rough sleeping, Kingston upon Hull City Council (in partnership with NHS Hull Clinical Commissioning Group) intends to bring all of these support services under one contract with a lead provider. This contract, 'The Hull Rough Sleeper Partnership', will be commissioned over the next twelve (12) months and will provide a flexible contract enabling joint bidding under new funding regimes to deliver new services as well as existing services.

Any contract let by Kingston upon Hull City Council for the purpose of providing support to rough sleepers or those at significant risk of rough sleeping from 1 May 2021 will include a mechanism for the contract to be novated from Kingston Hull City Council to the appointed provider of the Hull Rough Sleeper Partnership. Potential Providers for the Hull Homelessness Health Service therefore need to be aware that the commissioner and contractor will initially be Kingston upon Hull City Council however this will change during the contract and will become the Hull Rough Sleeper Partnership provider upon that contract being awarded.
