Description

Services offered under this Framework will support Beneficiaries with elements such as but not limited to: advice, support, data analytics, service mapping, system reviews, redesign,patient/user service development, service delivery provider audits and legal advice,
integrated working, and other collaborative working practices which may be required with either singularly or collaboratively with other publicly funded entities, either as a result of publicly funded entities being directed/mandated centrally to work together or as is determined and required by the Beneficiaries themselves.

The Framework is intended to provide a support and knowledge base for the emerging care and integrated care models encompassing vanguards, sustainable
transformation plans (STPs), multi-specialty community providers (MCP) joint service developments, primary and acute care systems (PACS), accountable care networks (ACN) or where organisations come together to explore new working opportunities. The framework is for the benefit of existing and emerging organisations during the life of the Contract, which includes but is not limited to providers, commissioners and support organisations.
