Description

GP Services - To provide medical input, care management, support and advice for the residents in Lings Bar
Rehabilitation centre, (72 beds). Ensuring the service is well led in-line with CQC domain definitions and fully supported to deliver high quality primary medical level of care and oversight for residents. Patients may have transferred from acute care or require step up from community-based care, are medically fit and require a period of assessment, rehabilitation recovery and/or reablement to achieve discharge into an appropriate community setting.
