Description

Provision of a Community Pain Management Service (CPMS) for the Bassetlaw area.

The aim of the service is to:

Provide clinical assessment, treatment and clinical leadership within a comprehensive community service.

Provide a community based pain management service using a robust triage single point of access model

Provide Tier 2 and 3 level services and advice to GPs who will deliver Tier 1

Provide a service which complies with latest guidance

Provide equity of access to service users

Provide access to a range of disciplines and interventions that can deal with the physical, psychological and social needs of the service users with chronic pain

Contribute to developing a local network for the management of chronic pain

Aim to ensure that service users experiencing chronic pain wherever clinically appropriate do not need to be managed in secondary care settings

Work in partnership with secondary care providers to identify further pathway development to reduce the need for treatment in acute settings. This includes access to diagnostics to enable appropriate triage of more complex cases

Refer / signpost to supporting services such as MSK CATS, IAPT services locally; build on local pathways and inter-referral to ensure patients get the care they require at a suitable time and at a suitable place

Accept referrals from a range of sources (where patient eligible) such as primary care and consultants an

Have in place a robust onward referral process to services such as MSK CATS, orthopaedics or rheumatology. Patients will be signposted and supported to contact other agencies such as IAPT, Social Prescribing or Exercise on Referral.
