Description

The current contract for the provision of GP cover and clinical service provision for Brighton House is a short-term service commissioned by the CCGs to assist with community bed capacity for low dependant patients. There may be a need in the future for other and/or additional GP providers to assist the local Community hospitals in similar ways. The current service aims to utilise access to GP services between the hours of 8:00am and 6:00pm, Monday to Friday, 52 weeks of the year. GP assistance is provided by primarily using on-site and off-site assistance including urgent provider response. On-site care expects a minimum of 2 hours direct patient contact each week incorporating a review of new admissions, at least one full ward round and ensuring care plans and appropriate discharge summaries are completed. The off-site care is a combination of telephone support and on-call services with necessary visits and assessments upon escalations by unit staff. Any treatments or management prescribed by GPs under these services need to be clearly and througherly explained to senior Trust staff and agreed upon so they are able to be fully supported in the treatment of patients. Any GP put forward under this or a similar contract is responsible for his/her own assessment, decision-making and compliance with provided Trust guidelines, while overall responsibility for the patient will remain with SSOTP. There is also a need to participate in the Multi-Disciplinary Team meetings (MDT) weekly for patient review.
