Description

The City Corporation is re-commissioning three of its early intervention and prevention services to support adults with additional needs, carers and people with memory or cognitive difficulties in the square mile, and will be doing so under one procurement exercise titled 'Reach Out Network".

Lot 1 (City Wellbeing and Independence Service (WISE) - To provide an early intervention and prevention service offering group and one to one support to City residents, particularly older residents and adults with additional needs, to help reduce social isolation. The service will help prevent the need for care and support by engaging with those residents not yet meeting the criteria for Adult Social Care support by encouraging and supporting people to actively participate in their community and to prevent social isolation. The service will work with partners to identify their need for support and liaise with partner agencies to make appropriate referrals where relevant to statutory and voluntary services.
Network Coordination -The City WISE lot will include the responsibility for coordinating the three services within the Reach Out Network. This will include ensuring:
o the Network services are integrated
o Outreach and promotion is coordinated
o Effective referral pathways between the Network partners and other services are established
o Lead on the strategic direction of the Network including supporting commissioners with coordination of the Services' Annual Service Plans.

Lot 2 (City Carers Service) -- To provide specialist independent help, advice and support for Adult carers living in the City of London Service to support them in their caring role. The Service will include group and one-to-one support sessions and other innovative models of support. Recognising the diverse needs of carers in the City of London, the City is looking for innovative and flexible proposals for carer identification and engagement providing support, including supporting carers who, male carers and BME carers. There would be an expectation that benefits advice and referral on would be a key element, via work with partners such as the City Advice Service, as well as referral to Adult Social Care for a formal carers Assessment for newly identified carers.

Lot 3 (City Memory Group) - To provide group and peer support for people with memory or cognitive difficulties or who have a diagnosis of dementia, their families and carers. Provision of structured and facilitated weekly support group sessions within an informal and relaxed environment to allow people to engage in meaningful activities that they are interested in; build self-esteem, assertiveness and confidence; support each other, share information and advice and access training.
