North Staffordshire and Stoke on Trent Clinical Commissioning Groups wish to commission, home care packages for adult patients who are in terminal decline at the end of their life or in a period of rapid deterioration, and are eligible for Continuing
Health Care services across Northern Staffordshire. For those patients with a rapidly deteriorating condition that may be entering the terminal phase, the Fast Track process was set out in the National Framework to enable immediate provision of NHS
Continuing Health Care. It is the CCGs intention to commission care that improves patient safety and quality of care for those who meet the criteria for Fast Track. Moving from the traditional commissioning on an individual case by case basis, CCGs
wish to commission, by means of a contract, a number of hours of home care per year specifically to support the speedy discharge of patients from a range of settings to enable patients to be cared for at home if this has been identified and documented as the preferred place of care and death. Providers will be required to deliver person centered care in line with national guidance and local requirements in a patient's own home, including but not exhaustive of assistance with: Medication, Nutritional management, Getting up/going to bed, Dressing, Washing and bathing, Continence management and aids, Grooming Listening and support with social and/or emotional needs. Contracts will be awarded to a maximum of two providers across the registered population of North Staffordshire and Stoke-on-Trent inclusive of rural locations.
