The contract is for the provision of a block volume of beds within care homes within the borough to ensure sufficient provision is available for those being discharged, or prior to hospital care to reduce and manage the need for acute care provision.Intermediate Care Services in Barnsley are provided as part of a formal Alliance Contract which consists of Barnsley Clinical Commissioning Group, South West Yorkshire Partnership Foundation Trust (SWYPFT), Barnsley Hospital NHS Foundation Trust, Barnsley Healthcare Federation and BMBC. Intermediate Care is part of the New Models of Care service delivery within the Barnsley geographical footprint.
