Description
The Support to Live at Home service is for: twenty four hour home care provision for older and disabled people (including those funded by Health for ongoing long term health conditions); end of life care at home; dementia care; and reablement. The service is divided into five separate geographical areas which will be tendered as four separate contract opportunities at different phases between 2015 and 2017 (see below for detail on the phases). he work does not include: provision for people with a learning disability; provision for people with poor mental health; or provision for people funded by health and social care due to complex needs. The Support to Live at Home contract will be a spot purchase contract. The geographical area in question for the delivery of home care comprises: Wraxall, Tickenham, Nailsea, Yatton, Congresbury Long Ashton, Failand, Backwell, Dundry, Felton, Winford, Redhill, Blagdon, Wrington, Langford, Churchill, Sandford, Winscombe. There are approximately 2900 home care hours delivered in this area currently per week.
