Description

NHS Erewash Clinical Commissioning Group, NHS Hardwick Clinical Commissioning Group, NHS North Derbyshire Clinical Commissioning Group and NHS Southern Derbyshire Clinical Commissioning Group seek to jointly commission a Continuing Healthcare Service.

Clinical Commissioning Groups (CCGs) have a statutory duty to meet the requirements of the Health and Social Care Act (2012) including the provision of NHS Continuing Healthcare and NHS Funded Nursing Care. This is undertaken by assessing, reviewing and case managing patients who are their responsibility where they have been identified as likely to be eligible for NHS Continuing Healthcare, or a joint funded package of care or are placed in a nursing home and are eligible for receipt of Funded Nursing Care.

The CCGs within Derbyshire are working on a 'Derbyshire wide footprint' for the commissioning of a Continuing Healthcare Service, however each individual CCG remains statutorily accountable for the provision of all elements of NHS Continuing Healthcare for its registered population.

The CCGs have taken the opportunity to review the current service specification and model with a view to making service improvements and recognise how important it is that stakeholders are informed and engaged in respect of this service.

The aim is to create a seamless health and social care service, which is patient focused, promotes independence, is delivered as close to home as possible, and to achieve a focus upon 'place-based systems of care' working in collaboration and partnership across organisations.

Derbyshire CCGs are therefore seeking to engage with suitably qualified service providers on the development of this service.

The CCGs have made available the draft service specification. There is an opportunity for service providers to articulate how they believe they can add value, support the development of the specification and outcomes to the current service, and to what extent can enable more effective integration with other parts of the system.

To support the engagement process Derbyshire CCGs plan to hold a 'Market Engagement Event' in the morning of Monday 4th July 2016 at: Babington Hospital, Derby Road, Belper, Derbyshire DE56 1WH. Please note places will be restricted to 2 persons per provider.

The purpose of the event will be to discuss at a high level the CCGs strategy and to use the knowledge of the market to help inform elements of the service specification. The procurement process and timetable will also be shared.
